The Christ Hospital is pleased to offer MyChart, which provides you direct online access to portions of your electronic medical record. To access The Christ Hospital MyChart, you must agree to be bound by the terms of this MyChart Terms and Conditions agreement for the material and data contained in MyChart. Please read the following information carefully. This Terms & Conditions agreement outlines our practices, your responsibilities and our sensitivity to your right to privacy. We reserve the right to revoke access at any time for any reason. If you do not agree to the terms of this agreement, do not use The Christ Hospital MyChart, and promptly exit this site.

What is MyChart

The Christ Hospital MyChart provides you with secure web-based access to parts of your electronic medical record, where your doctor stores your health information. Lab results, appointment information, medications, immunizations, and more are all securely stored for quick retrieval. The Christ Hospital MyChart also provides convenient methods of communicating with your doctor’s office, allowing you to renew prescriptions, send messages and schedule appointments online. Access to The Christ Hospital MyChart system is granted to patients for the purpose of furthering their medical care. Use of the system unrelated to this purpose may result in the loss of access to MyChart.

Medical Information

Information on The Christ Hospital MyChart is provided "as is" without warranty of any kind and is meant for use only to support the patient's relationship with their current or future medical professional. Information provided on this site does not constitute medical advice. Relying on the material in The Christ Hospital MyChart is at your own risk. You should consult with a physician or other healthcare provider regarding your own condition and how this site content may or may not apply to you. You may be accessing your own medical information via the MyChart application and may wish to communicate with one of your healthcare providers about that or related information. Neither MyChart nor any other application available through this site is a substitute for appropriate and timely contact or consultation with your physician. You should never change or stop any course of treatment prescribed by your physician without first consulting him or her.

MyChart User Responsibilities

By using The Christ Hospital MyChart, you agree to all the terms and conditions of this agreement and our posted Notice of Privacy Practices, found at www.thechristhospital.com. We may modify this agreement and the Privacy Notice at any time, and you agree to those changes as soon as they are posted. You should review this agreement and the Privacy Notice from time to time so that you are aware of all changes.

You understand that The Christ Hospital cannot guarantee the accuracy or correctness of the site. There may be typographical errors or other mistakes. You agree to contact your physician’s office and complete an amendment request form if you find errors in your medical or health care information.

The Christ Hospital assumes no responsibility for how you use the information you obtain from this site. When you download or view information on The Christ Hospital MyChart, you agree to hold The Christ Hospital, its subsidiaries and affiliates harmless and will not hold them liable for any damages or claims arising out of, or in any way connected with, your use of MyChart or the information contained on the site.

The Christ Hospital MyChart may offer links to other healthcare Web sites not managed by The Christ Hospital. These sites are informational only; The Christ Hospital does not endorse and has not verified the accuracy of the information in/on these sites, and you should not rely on any of the information on the Web sites for medical diagnoses or treatment. You should never change or stop any course of treatment prescribed by your physician without consulting him/her.

MyChart information and logos, icons and other marks are reserved by EPIC and The Christ Hospital and are protected by copyright and trademark law. You agree that you will not reproduce any information without written consent of EPIC and The Christ Hospital.

You understand and agree that you are solely responsible for any sharing of The Christ Hospital MyChart information that you intentionally or unintentionally communicate to others, and that The Christ Hospital and it’s affiliates shall not be liable for any damages suffered by you or third parties related to the material and information in MyChart.

All communications between you and healthcare providers using The Christ Hospital MyChart occur over a secure connection. You are required to provide The Christ Hospital with a valid e-mail address, and it is your responsibility to keep this address updated with MyChart if you change your e-mail account.

You understand that The Christ Hospital takes no responsibility for, and disclaims any and all liability or damages arising from a breach of medical record confidentiality resulting from your sharing or losing your password. If The Christ Hospital discovers that you have inappropriately shared your password with another person, or that you have misused or abused MyChart access privileges in any way, your participation in The Christ Hospital MyChart may be discontinued by The Christ Hospital without prior notice.

You agree that you will not post any offensive material on this site. If we determine that you have violated this agreement or abused the use of this service, we reserve the right, at our sole discretion, to terminate your use and access of The Christ Hospital MyChart. You and your healthcare provider will be notified if your MyChart service has been discontinued.

Availability & Eligibility for MyChart Account -

An individual must be age 18 or older to request a personal account. Parents/guardians of children under the age of 18 may request proxy access for their children. To initiate proxy access, contact the provider’s office for the appropriate forms.

MyChart may not always be available to you due to system maintenance, back-ups or failures. Access is provided on an "as is available" basis, and you are not guaranteed access at any particular time. If access is restricted for planned maintenance of the MyChart site, reasonable efforts will be made to communicate downtime to users in advance. However, there may be reasons to restrict access to The Christ Hospital MyChart without advance notice.

Activation of MyChart

When you decide to use MyChart, you will begin the enrollment process at your physician’s office. You will be asked to verify your identity in person or by signature comparison at that time. You will then be provided with an access code which will expire 60 days after you receive it. The access code you receive will give you the opportunity to create a personal login name (called MyChart ID) and a password. Once this access code has been used, it will no longer be valid. It is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access your confidential medical information. That person will also be able to read your messages and send new messages as if that person were you. It is your responsibility to keep your login and password confidential, and to change your password if you feel that its security has been compromised. You may change your password at any time.

Loss of Access to MyChart

The Christ Hospital may restrict or terminate access to MyChart at any time. Each circ*mstance will be evaluated and decided individually. The following list includes, but is not limited to, reasons which will result in loss of access: The Christ Hospital Health Network may terminate or revoke access to MyChart at any time. Each circ*mstance will be evaluated and decided individually. The following list includes, but is not limited to, reasons which will result in loss of access for a patient and/or patient/proxy:

1. The patient and/or patient proxy is not following specific terms and conditions for use of the application.

2. The patient and/or patient proxy threatens TCHHN staff, providers or management. You are no longer a patient of The Christ Hospital.

3. The patient and/or patient proxy’s provider believes information available in MyChart will negatively impact patient care.

4. The patient and/or patient proxy has inappropriately shared his/her password with another person.

5. The individual is no longer a patient of The Christ Hospital.

6. The patient and/or patient proxy has had a number of failed login attempts to access MyChart.

7. The patient and/or patient proxy has accessed other patient’s MyChart accounts without their consent. The patient and/or patient proxy has had a number of failed login attempts to access MyChart.

In addition, The Christ Hospital Health Network may restrict some access to view only at any time. Each circ*mstance will be evaluated and decided individually. The following list includes, but is not limited to reasons which will result in restricted access for a patient and/or patient proxy:

1. The patient and/or patient proxy cancels appointments frequently via MyChart.

2. The patient and/or patient proxy frequently reserves appointment times, via MyChart, that are not needed.

3. The patient and/or patient proxy is unable to use MyChart features without constant assistance.

4. The patient and/or patient proxy is using inappropriate language when communicating with staff or providers via MyChart.

5. The patient and/or patient proxy refuses requests to make an in-person appointment to discuss their issue.

6. The patient and/or patient proxy repeatedly sends inappropriate messages regarding urgent issues or issues that are unrelated to his/her healthThe patient and/or patient proxy has had a number of failed login attempts to access MyChart.

7. The patient and/or patient proxy has engaged in other activities which disrupt office or department operations.

Response to Electronic Communication

The Christ Hospital will make its best effort to provide a timely response to electronic inquiries. In some cases, the office staff that needs to respond to an electronic inquiry or other communication may not be immediately available, so patients should allow at least 48 business hours for a response. Accordingly, emergency situations requiring immediate attention should not be submitted electronically. Contact your office, your physician or 911 immediately if you are suffering from urgent medical issues.The Christ Hospital is only able to respond to electronic communications sent by the patient based on the information provided by the patient. If there is insufficient information provided, the hospital will be unable to provide accurate and reliable responses.

E-mail Privacy -

You will be notified via e-mail when there is new medical information to be viewed on your MyChart account. This means that any person with access to your e-mail will be able to see this notification. This could include your spouse, employer or anyone else that can access your e-mail account. Although no private medical information will be sent in this e-mail, this notification that new medical information is available by accessing MyChart may be information that you would not want others to know. You should take this into account when providing an e-mail address.Please know that if you send an e-mail communication via MyChart, it may be shared with Christ Hospital staff that assist the physician in providing your medical care. Your confidential medical information on The Christ Hospital MyChart will be accessible only to clinical and administrative staff who need the information to perform their jobs.

Security and Confidentiality

All of the confidential medical information available to you in The Christ Hospital MyChart is protected and securely maintained by The Christ Hospital. The Christ Hospital is committed to protecting the confidentiality of your medical information on MyChart in the same way we protect medical information throughout the organization. We limit Christ Hospital employees and affiliates access and ability to enter or view information based upon their role in your care.

The Christ Hospital utilizes industry accepted methods and technical security measures to protect the confidentiality and integrity of your information in much the same way as that of online banking. As such, access and activity of your MyChart information is recorded. If, during routine reviews of such audit trail information, The Christ Hospital suspects or has any concerns about the confidentiality or integrity of your information, The Christ Hospital reserves the right to suspend any and all access to that information and will make attempts to contact you immediately.

The Christ Hospital may collect information you provide about yourself through the forms offered on our site. This is to provide personalized services, or communicating separately with you. This personal information is not shared with any third party outside of The Christ Hospital.

The Christ Hospital may also collect information about your visit to our MyChart web site such as the date and time of your visit, the pages of our site you visited, and your IP address of origin. We use this information for statistical purposes to help us understand the effectiveness of our web site and how we can improve it.

The Christ Hospital places considerable emphasis on the privacy of patient information and its compliance with State and Federal Privacy Laws. MyChart is configured to be secure from unauthorized access, but The Christ Hospital cannot be held responsible for:

a) Absolute security of all electronic communication transmissions between patient and clinic;

b) Unauthorized disclosure resulting from a user not logging out of an active session;

c) Unauthorized disclosure resulting from a lost or stolen user ID and password;

d) Unauthorized disclosure resulting from information printed by the user from MyChart;

f) Unauthorized disclosure resulting from personal computer settings or installed software products that may compromise information security; or

e) Similar events outside the control of The Christ Hospital

Copy of Complete Medical Record

The electronic record in The Christ Hospital MyChart is the property of The Christ Hospital and the information displayed in MyChart is only a portion of your complete medical record. Contact your physician’s office directly for a copy of your medical record.

By agreeing to these terms and conditions, I authorize The Christ Hospital to release my personal health information to me via MyChart.

General

Governing Law and Dispute Resolution

This Agreement shall be governed by the laws of the State of Ohio and any dispute arising hereunder must be submitted to binding arbitration in Ohio, pursuant to the Rules and Provisions of The American Health Lawyers Association

Entire Agreement

This Agreement contains the entire agreement between the parties with respect to the subject matter contained herein and it shall not be modified or amended except by signed written instrument. The terms of this Agreement may be changed at any time in the discretion of The Christ Hospital and updated terms will be posted on the MyChart website for user access.

Other Uses of MyChart Information

I understand that from time to time I may be asked to complete patient satisfaction surveys via MyChart. The Christ Hospital may analyze information submitted via MyChart as part of descriptive (demographic) studies and reports. In such cases all of my personal identifying information will be removed.

Fees

The Christ Hospital reserves the right in the future to charge a fee for the use of MyChart. You will be notified of this change, should it occur, and asked to re-apply if you agree to the fee and wish to continue using MyChart.
